New Patient Registration

You or your nominated representative (in the case of children, disabled or infirm) can register with our Practice by completing the appropriate form, which is available by request from the receptionist.

Babies

You can register your newborn baby as a patient at our practice as soon as you have registered his/her birth at the registry office. Please bring the white form (FP58) which was given to you there.

Out of Area Registrations & Practice Boundary

As part of the programme to extend patient choice, NHS England rolled out a service nationally whereby a patient may register with the doctor’s Practice of his or her choice, no matter where that patient lives. The only drawback with this arrangement will be that, the doctor from the patient’s registered Practice will not make any home visits to any patient outside of the Practice Boundary, as indicated by the red line on the map displayed here. All required home visits will require for the patient to make the necessary arrangements to access such care by contacting 111 and be guided to the relevant services that have been commissioned.

It is therefore strongly recommended that patients may find it preferable to register with a Practice near their home so that if they ever do require a home visit, they will be able to be seen by one of their own regular doctors from their own Practice.
